Memory Care Coordinator for Assisted Living

Port St Lucie, Fort Pierce, Stuart, FL

We are seeking a compassionate and organized professional to help coordinate resident-centered memory care services in an assisted living setting. In this role, you will support daily operations, promote a safe and engaging environment, and collaborate with team members to help residents living with memory loss receive respectful, individualized care.

Responsibilities

  • Coordinate day-to-day memory care programming and support services for residents.
  • Help develop and maintain individualized engagement plans based on resident needs, preferences, and abilities.
  • Collaborate with nursing, caregiving, activities, and leadership teams to support quality resident care.
  • Monitor resident participation, well-being, and changes in condition, and communicate observations to appropriate team members.
  • Support a positive, structured environment that encourages dignity, independence, and meaningful engagement.
  • Assist with family communication, care conferences, and ongoing updates as appropriate.
  • Help ensure compliance with applicable policies, procedures, and care standards.
  • Provide guidance and support to care staff regarding dementia-informed approaches and resident interaction.
  • Maintain accurate documentation related to resident programming, support needs, and coordination activities.
  • Participate in training, team meetings, and continuous improvement efforts.

Qualifications

  • Experience in assisted living, memory care, senior living, or a related healthcare environment.
  • Knowledge of dementia care principles, resident engagement strategies, and person-centered approaches.
  • Strong interpersonal and communication skills with the ability to work effectively with residents, families, and team members.
  • Excellent organizational skills and attention to detail.
  • Ability to handle multiple priorities in a compassionate and professional manner.
  • Basic computer skills and familiarity with documentation systems.
  • Ability to maintain confidentiality and exercise sound judgment.

Preferred Qualifications

  • Experience coordinating activities, resident programs, or care services in a memory care environment.
  • Training in dementia care, gerontology, social services, recreation therapy, or a related field.
  • Current certifications or credentials relevant to assisted living or senior care, if applicable.
